It's pretty good. Treasure rooms have better items, secret rooms have worse items. The new characters are a second-rate Isaac and a short-range powerhouse. Some rooms repeat themselves a bit too often but not as often as they did in the original.

Ah, and there is a super-secret character that was going to require some ARG-like effort to unlock, but someone just hacked the game and found out how to do it. McMillen was pissed.

The community had basically found it all out anyway a day or so after release. You had to die a certain way and you'd get one of ~200 puzzle pieces that spelled out the final solution. They had reached that part and were already assembling them when someone went "found it" via datamining.

So at best the ARG would have lasted one more day.
